Haus of Transcendent (HOT), a leader in providing care coordination for the LGBTQI2S+ community in Greater Cleveland, is proud to announce it has received funding from The Cleveland Foundation to support our programs and initiatives. This generous investment will directly advance HOT’s mission to deliver essential, coordinated care and build a more inclusive and healthy community.

The funding will help HOT provide specialized care coordination services for LGBTQI2S+ individuals and allies through its role as a care coordination agency of the Better Health Partnership Pathways HUB. HOT’s model focuses on the basics: reduce harm, connect to care, and dream big. As the first Cleveland nonprofit run by and for LGBTQI2S+ people to join the Pathways HUB, HOT provides vital case management to Medicaid recipients in Cuyahoga County.HOT’s no-wrong-door approach to case management ensures that people without insurance have access to HOT’s multilingual care coordination services.

HOT was also selected as a recipient of The Cleveland Foundation’s LGBTQ+ Opportunity Fund. This grant will launch two important initiatives that extend HOT’s leadership beyond direct service:

  1. The HEAL Directory: HOT will create the HEAL (Health Equity Accessibility Language) Directory, a comprehensive healthcare resource for Cleveland. It will list providers offering essential services—including STI screenings, treatment, vaccines, and primary care—for Medicaid recipients and uninsured folks who depend on sliding fee scale. To maximize accessibility, the directory will be available in Queer-friendly Spanish and Arabic.
  1. LGBTQ+ Cultural Competency Training: HOT will develop and implement a professional training program focused on language access and accessibility. This initiative will equip healthcare and social service providers across the city with the skills to create more welcoming, effective, and equitable environments for LGBTQI2S+ individuals.

This has been an incredibly challenging year for Transgender people, LGBTQI2S+ immigrants, and people with disabilities,” said Jordi Luke, CEO of HOT. “The Cleveland Foundation’s support allows us to help more members of our community by removing the systemic barriers that stand between them and their health and wellbeing.”
