HOT Welcomed as New Better Health Partnership Pathways HUB Care Coordination Agency

At Better Health Partnership, we know that unmet social needs are not confined to any one group—they affect people across all types of insurance coverage, income levels, and backgrounds. Too often, it is assumed that social barriers to health primarily impact those who are uninsured or covered by Medicaid. However, research continues to show that individuals with Medicare, employer-sponsored insurance, and ACA Marketplace plans also face significant challenges accessing food, housing, transportation, and other essential resources that shape their health. If we are serious about improving population health, we must address these needs broadly and equitably.

This is why we are excited to share the expansion of our Pathways HUB as we launch a Medicare pilot in partnership with Medical Mutual of Ohio. Additionally, we are honored to welcome Benjamin Rose Institute on Aging and Haus of Transcendent (HOT) as new care coordination agency partners that provide innovative solutions to populations that have long faced barriers to care. These collaborations reinforce the power of community-based solutions that are tailored to the unique needs of older adults, LGBTQI2S+ individuals, and others who have historically been marginalized. The HUB continues to evolve as a trusted bridge between healthcare and social services, ensuring that more people receive the support they need to achieve better health.

Haus of Transcendent (HOT) Welcomed as New Better Health Partnership Pathways HUB Care Coordination Agency
Better Health Partnership is excited to welcome Haus of Transcendent (HOT) as its newest care coordination agency. HOT connects LGBTQI2S+ adults to community resources that address social drivers of health.

“Haus of Transcendent (HOT) is thrilled to join the Better Health Pathways Hub as the first LGBTQI2S+ led and serving organization in this vital network,” said Jordi Luke, Chief Executive Officer. “Community health workers (CHWs) play a critical role in identifying and addressing the social drivers of health, making the HUB a natural alignment with HOT’s mission to create an accessible, vibrant, and healthy community for all. This partnership allows us to expand our impact, strengthen care coordination, and reduce harm for the LGBTQI2S+ individuals and allies we serve. We are grateful for this opportunity to improve the health of our community!”
