It will be comprised of two affordable housing buildings and a community room, with wraparound support services provided from our on-site Resource Hub, a newly acquired 8,000 sq. ft. building located directly across the street from our housing facilities. As a Better Health Pathways HUB Care Coordination agency, our Community Health Workers specialize in connecting the community to critical resources: physical and behavioral healthcare, social services, legal help, food, transportation, employment, and more. This holistic approach is designed to provide both short and long-term stability.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0d52065
